First-Time Passport Application

Adults (age 16 and older)

All applicants must apply in person.

  • Present proof of U.S. citizenship
  • Present one valid proof of ID
  • Provide one passport photo
  • Complete form DS-11

Children Under 16

  • All applicants (children) must appear in person accompanied by both parents.
  • Present proof of U.S. citizenship
  • Present proof of parent-child relationship
  • Parents’ valid IDs
  • Provide one passport photo
  • Complete form DS-11
  • Submit Notarized DS-3053 (PDF – 157 KB)if either parent cannot be present. Also bring a photocopy of the non-applying parent’s ID.

Passports for Children

As of February 1, 2008, the two-parent consent requirement for all minors’ passport services was extended to include all children under age 16. Both parents are required to be present to provide signatures at the time of application. See: Parental Conset for Minors
